From the main menu, hover over the "Surveys" icon, and click on "App Settings"

Choose "+ New Survey"

In the fields provided, title your survey and write a short description.

Clicking the "+ New Section" button will create a new section for your survey that you can title. Clicking the "+ New Question" button on the right of a section title will produce a pop up, in which you can write your question or statement. Remember, your client or customer will be answering on a scale of 1-5.

To view the results that have been submitted, go to the main App Settings page and click "View Results" in the top right. From here you can select who's results you would like to view. If one user has completed several surveys, they will all show under that user's name.

Surveys will allow you to create as many surveys as you like on a single site.
